Long before nano-hydroxyapatite (nanoHA) became a popular topic online, we worked to provide our patients with product recommendations that were effective, biocompatible, and supported by science.
As more high-quality clinical studies have been published, nano-hydroxyapatite has emerged as one of the most promising fluoride-free ingredients available for reducing tooth sensitivity and supporting the natural remineralization process. That growing body of evidence has reinforced our confidence in recommending nanoHA as an effective part of a fluoride-free approach to better oral health.

Most patients who choose Nashville Restorative Dentistry have already decided they prefer fluoride-free oral care. They aren’t asking us whether they should avoid fluoride—they’re asking what fluoride-free products actually work.
Nano-hydroxyapatite has been studied extensively, and the research continues to grow. While no toothpaste or product can replace a healthy diet, ideal plaque control, healthy saliva flow, and regular professional care, nano-hydroxyapatite has become one of the ingredients we feel most confident recommending to patients who want a fluoride-free remineralization option backed by legitimate research.
Hydroxyapatite is the primary mineral that makes up your tooth enamel and dentin.
Nano-hydroxyapatite is simply hydroxyapatite manufactured at an incredibly small size, allowing it to interact more effectively with the tooth surface. Because it closely resembles the mineral naturally found in enamel, it can help restore areas where enamel has been weakened by the demineralizing effects of acid.
Rather than simply coating the teeth, nano-hydroxyapatite supports the body’s natural remineralization process by helping restore calcium and phosphate to weakened enamel.

One important point often overlooked online is that not every nano-hydroxyapatite toothpaste is the same.
Research suggests that particle size and shape influence how effectively nano-hydroxyapatite interacts with tooth enamel, as well as whether or not it could be cytotoxic (damaging to cells). Rod-shaped particles most closely resemble the natural crystal structure found in healthy enamel and are generally considered the preferred shape for both effectiveness and safety. Needle-shaped nanoHA particles are concerning as a potential source of cell and tissue damage because of their potential to cross cell membranes or enter tissues.
Unfortunately, many products marketed online provide little information about where their nano-hydroxyapatite comes from or how it is manufactured. That’s one reason we encourage patients to choose professionally recommended products from companies that are transparent about their manufacturing standards.

CariFree’s fluoride-free tooth gel contains 2% nano-hydroxyapatite, specifically nanoXIM® by FLUIDINOVA®. FLUIDINOVA is a company in Portugal that specializes in medical- and dental-grade calcium phosphate biomaterials and manufactures a high-purity, rod-shaped nano-hydroxyapatite that closely resembles the natural crystal structure found in healthy enamel.
What gives us additional confidence is the level of scientific scrutiny behind this nanoHA source. The Scientific Committee on Consumer Safety (SCCS) is an independent scientific committee that advises the European Commission on cosmetic ingredient safety, and it has conducted extensive reviews of rod-shaped nano-hydroxyapatite. After evaluating toxicology data and manufacturing specifications over multiple submissions, the SCCS concluded that qualifying nano-hydroxyapatite is safe for use in toothpaste at concentrations up to 29.5% and in mouthwash at concentrations up to 10%.
CariFree’s fluoride-free tooth gel contains 2% nano-hydroxyapatite, well below the SCCS’s maximum allowable concentration for toothpaste. More importantly, it uses the same nanoXIM that has undergone this extensive review and is the only nanoHA approved for oral care by the SCCS.
To us, that’s meaningful. We don’t recommend products because they’re trendy—we recommend products that are supported by good science, manufactured to high standards, and consistent with our philosophy of helping patients achieve excellent oral health using biocompatible, evidence-based products.
